Enrollment Form
This form is the first form we need to start the registration process. It asks for valuable information that the school must have on file for your child's safety. We do not give out any of this information except what we place in the school directory.
Registration Information
This from is to be filled out by parents of new students. It give information we need for the cumulative and permanent files.

This form is for added information to apply the correct placement of services for a new student.
Fees Form
This form is needed by all parents. Please fill out the portions you may need. The office can help with any questions you may have as to which fees you owe.
Plegde of Support & Family Values
These two forms are for all parents to consider and sign. It speaks to the obligations of the parent and school, and the values we hold as Christians.
Release of Records
This form is for a new student transferring from another school.
Referral Form for Kindergarten - 2nd Grade
This form is to be filled out for students entering Kindergarten through second grade here at Our Savior.
We request that you take this form to your previous school and have the teacher and/or Principal fill out the form for our information. This information will be kept in the strictest of confidence.
Referral Form for 3rd - 8th Grade
This form is to be filled out for students entering Third through Eighth grade here at Our Savior.
We request that you take this form to your previous school and have the teacher and/or Principal fill out the form for our information. This information will be kept in the strictest of confidence.
Discretionary Form
This form is for families who wish to lower their tuition. We ask that all information be given in complete with verify all lines. This information is not stored or kept permanently and is only used to determine qualifications. This information is kept strictest of confidence.
Tuition Agreement Contract
This form is for the tuition families to fill out once you know the cost of fees and tuition. Fill out and send to school or bring it to the office for verification and signatures.
Student Medication Form
Parents, we request that you fill out this form to tell us of any medication your child is taking at home or at school that may affect ones matters at school.
This form also allows for parents to give permission to take over-the-counter medications by way of a phone call or signed permission on a note or signed permission on this form.
The signed permission is only good for the current school year.
Medical Release Form
This form is used when there is a medical emergency, and the student needs to be released to medical personnal for attention. This in no is a blanket permission slip for fieldtrips, but permission to treat in case of accident here at school or while on a school activity away form the building.
Photo Release Form
This form asks for permission to use the school's photographes of you child in presenting the school in its activities and classrooms.
School Supplies List
This form will show you the items that each child will need in any class. Please look over the list and shop during summer sales.
Blank Permission Slip
This form is a general (blank) permission slip that can be used by a parent if the original slip is misplaced.
