This form is to be filled out by all parents.  The form is for permission to do background checks for volunteering at school.

This form is the first form we need to start the registration process. It asks for valuable information that the school must have on file for your child's safety. We do not give out any of this information except what we place in the school directory.

This form is for a new student transferring from another school.

This form is to be filled out for students entering Kindergarten through second grade here at Our Savior.

We request that you take this form to your previous school and have the teacher and/or Principal fill out the form for our information.  This information will be kept in the strictest of confidence.

This form is for families who wish to reduce their tuition.  We ask that all information be completed  with copy of receipts to verify all lines.  This information is not stored or kept permanently and is only used to determine qualifications.  This information is kept  in strictest confidence.

This form is for the tuition families to fill out once you know the cost of fees and tuition.  This form will usually be completed in the office. Once verified  and signed, this will set the tuition and fees for the school year.

Parents, we request that you fill out this form to tell us of any medication your child is taking at home or at school that may affect ones matters at school.

This form also allows for parents to give permission to take over-the-counter medications by way of a phone call or signed permission on a note or signed permission on this form.

The signed permission is only good for the current school year.

This form will show you the items that each child will need in any class.  Please look over the list and shop during summer sales.

This form is a general (blank) permission slip that can be used by a parent if the original slip is misplaced.
